To become a SPARC member, all you need to do is two things:

1) Come into the dispensary at 1256 Mission Street with your doctor’s recommendation and a government issued ID or drivers license. (Note: You do not need a medical marijuana patient ID card to become a member – just bring in the original copy of your doctor’s recommendation.)

2) Fill out a registration form.

After we verify your recommendation and receive your registration form, we will give you an official SPARC membership card. And that’s it – you are officially a SPARC member!

We ask that new patients please arrive a half hour before closing to allow for extra registration time.

Designated Caregivers:

Designated Caregivers are permitted at SPARC with the following conditions:

1. All caregivers must have a verifiable document that designates them as a caregiver to a patient. Caregivers without such a document will not be allowed inside SPARC.

2. Caregivers and the patients they represent must fill out and sign the standard SPARC member agreement and the SPARC Patient and Primary Caregiver form (available for download here). Forms must be signed by BOTH patient and caregiver.

3. All caregivers must bring their own state issued ID as well as a state issued ID of the patient they represent (an appropriate copy is acceptable).

Patients need not be present during the caregiver signup process but a caregiver without the documents listed above will not be allowed in SPARC. For questions please email info@sparcsf.org or call 415.252.7727.
